Details for Procuring an Oxygen Concentrator

The following documents are mandatory for procurement of the oxygen concentrator from the One Consciousness Society Foundation [“OCS Foundation”]:

1. Copy of a Government issued identity card of the applicant [the document should contain the applicant’s full address as well as contact details];
2. A positive RTPCR Report from a recognised laboratory that should be at most from seven days before procurement of the Oxygen Concentrator;
3. A prescription from a registered medical practitioner stating the requirement of an oxygen concentrator;
4. Call / message from reference from OCS Foundation
5. A signed copy of the below mentioned declaration. (email will also work)

Declaration for Issue of Oxygen Concentrator

•  I understand that the oxygen concentrator is being provided to me by the OCS Foundation for a period of three [4] days. I state that I shall not hold the OCS Foundation responsible for any loss or damage caused due to the use of the oxygen concentrator provided to me.
• I state that the oxygen concentrator shall be used by me under the supervision of a registered medical practitioner. I shall not hold the OCS Foundation responsible for any issues medical or otherwise that may arise due to my usage of the oxygen concentrator.

• I shall consult a registered medical practitioner before using the oxygen concentrator.

• I understand that this is an electronic machine that is being provided to me by the OCS Foundation and declare that I shall not hold the OCS Foundation liable for any malfunction of the oxygen concentrator due to any factor, including but not limited to mishandling, power outage, etc. I state that I shall not rely on the oxygen concentrator solely and make appropriate arrangements in case of any malfunction.

• I understand that the oxygen concentrator has been provided to me for a period of [4] Four days and in case I require to extend my use of the oxygen concentrator, I shall give prior notice of 24 hours to the OCS Foundation. The OCS Foundation may or may not, in its sole discretion, permit me to keep and use the oxygen concentrator after the initial [4] four days period.

• I understand that I shall apply utmost due diligence to maintain the oxygen concentrator while it is in my possession, I shall endeavour to deliver it to the OCS Foundation in the same condition as I had received it. In case I am unable to deliver it in the condition as I had received it, I understand that I shall be liable to pay damages to the OCS Foundation.

• I completely understand that the oxygen concentrator is being provided to me on an ‘as is’ basis and that the OCS Foundation does not guarantee or make any claims as to the functionality, suitability, quality, fitness, etc. of the oxygen concentrator. I completely understand that the OCS Foundation is not the manufacturer of the oxygen concentrator.

• I understand that it is my sole responsibility to ensure that the oxygen concentrator provided to me by the OCS Foundation is in working condition. By accepting the oxygen concentrator, I accept the condition it is delivered to me in as well as the terms of the present declaration.

• I hereby confirm that I shall not bring any claims or initiate any legal proceedings, either criminal or civil, against the OCS Foundation, or its members, for any reason whatsoever in relation to my procurement and subsequent use of the oxygen concentrator.
• I understand that the terms of the declaration are subject to change by the OCS Foundation and it is my duty to check www.ocspeople.org for the latest updated version of the terms of the declaration.
